A renal autotransplant is the treatment option we provide for Nutcracker Syndrome and Loin Pain Hematuria. This surgery entails denervating and cutting the left renal vein and artery via a staple device and reimplanting/reattaching the kidney and placing it a couple of inches lower on the vena cava and aorta while leaving the ureter attached.
WHAT IS NUTCRACKER SYNDROME?
Nutcracker Syndrome is the most common diagnosis that requires an Autotransplant and is a compression of the left renal vein between the superior mesenteric artery and the abdominal aorta. This compression could cause symptoms such as constant left flank pain, headaches, pelvic pain, hematuria, amongst others. Each patient is different and might experience slightly different symptoms.
WHAT IS LOIN PAIN HEMATURIA?
Loin Pain Hematuria causes symptoms such as right sided flank pain and the presence of blood in urine. Each patient is different and might experience slightly different symptoms, most do not have a compression.

This technique includes a standardized, work up for each patient to determine whether surgical intervention is the best option for them, and not cutting the ureter or placing a stent during surgery.

Prior to surgery, we require specific testing/appointments to be completed. Please note, that we are unable to order these for new patients and require your referring physician to. We require all imaging discs and reports to be mailed to our office. Steps 1-4 must be fulfilled before an initial consultation can be scheduled with Dr. Zendejas.
1. Referral from your physician
for the treatment of Nutcracker Syndrome or Loin Pain Hematuria Syndrome.
2. Copies of the front and back of your insurance card
(We are contracted with most insurances; however, please refer to our “insurances we accept page to ensure we accept yours.)
3. Triphasic CT of your abdomen and pelvis
(We are contracted with most insurances; however, please refer to our “insurances we accept page" to ensure we accept yours).
4. Venogram - including pressure gradients and long run-off
(This is a test done through interventional radiology where dye is injected through your neck into your renal vein to show compression and collaterals.)
5. Renal Block
(This is a procedure also done by interventional radiology where they numb the nerves around your left or right kidney (depending on where your pain is) to determine if an Autotransplant would relieve your symptoms.)
6. A urology, nephrology, and/or pain management consultation
may also be required depending on the protocol your insurance has in place for authorizing an Autotransplant.
